How to use this worksheet

Use the real review date and its source. Enter one goal per record, with a measure and the resources needed. Keep delivery and feedback separate, and record when a goal changes instead of silently replacing the old expectation.

Worked example

Example: “improve quality” lacks a measure. Record the wording, ask what sample, error definition and review period apply, and note the response. Attach no files here: reference the work and feedback records you can lawfully retain.

What to do next

The result is a factual comparison and meeting preparation record, not a prediction of confirmation or dismissal. Acknowledging receipt and agreeing with an assessment are different fields. The 30/60/90-day template is a planning aid, not a statutory probation limit.
