How to use this worksheet

Write the requested outcome before adding the timeline. Separate things you personally observed from reports and interpretations. Reference existing documents by a short label; keep the originals separately. Missing evidence stays visible as missing.

Worked example

Example: a meeting invitation mentions performance concerns without examples. Record the invitation date and the wording you can verify. Ask which work, standard and review period are involved, what resources are available and when the next review occurs. Do not invent a witness or attachment.

What to do next
